Thank you for attending the 2009 National Primary Oral Health Conference. Your participation contributed to the success of our event and we truly appreciate your time.  Please note the following information on CE credit:

To complete your CE evaluation at a later date, please visit: www.ceevaluations.net and follow instructions.  

If you did not complete your CE credit evaluations during the conference or need to print your statement, click on the link above.

PLEASE NOTE:

Once you print your statement, you will not be able to make any additional changes.  If you have questions about your CE number, contact Luana Harris-Scott at adminsupport@nnoha.org. If you need to make a correction to a completed certificate, call the CE Evaluation office at 217-529-6941 and ask for Bonnie Chandler or Regina Benedict.
