Evaluations/CE Credit

Thank you for attending the 2009 National Primary Oral Health Conference. Your participation contributed to

the success of our event and we truly appreciate your time. Please note the following information on CE credit:

To complete your CE date at a later date, please visit: www.ceevaluations.net and follow instructions.  

If you did not complete your CE credit evaluations during the conference or need to print your statement, click on

the link above.

You will have until December 14th, 2009 to complete your CE requirements.

The initial screen will prompt you to enter your CE number (located on your conference name badge).

After entering the CE system, simply follow the on-screen directions.  You will need to complete an evaluation

for each session in which you wish to receive credit as well as the overall conference evaluation. You can

enter/exit as many times as necessary until you print the certificate.  Once you complete the needed evaluations,

click “View Your Submissions/Print CE”. From here you will be able to review your CE statement. If you need

to make changes or complete additional evaluations, simply hit the 'back' button.   This will take you back to the

evaluations section.  If you do not need to make any changes/additions, hitthe “Print CE Statement”.  Please note,

once you print your statement, you will not be able to make any additional changes.  If you have questions about

your CE number, contact Luana Harris-Scott at adminsupport@nnoha.org. If you need to make a correction to a

completed certificate, call the CEevaluation office at 217-529-6941 and ask for Bonnie Chandler or Regina 

Benedict.
