Position Overview
Position Title: Community Health Worker
Department: Perinatal Health and Wellness Program (PHWP)
Reports To: Arelis Martinez, PHWP Director
Role Type: Non-Clinical
FLSA Classification: Non-Exempt
Employment Type: Part-Time — 25 hours per week (flexible)
Pay Rate: $21.00 per hour
Primary Locations: KSCOHC Bloomfield; outreach, educational, maternal health, tabling, and recruitment events representing KinderSmile Foundation and PHWP
Position Summary
As a KinderSmile Foundation Community Health Worker, you will work closely with Dentists, Dental Hygienists, Dental Assistants, Front Desk Patient Coordinators, and other Dental Home and Foundation staff and volunteers, fostering a team-oriented atmosphere and furthering the culture of KinderSmile Foundation.
The Community Health Worker is an integral part of KinderSmile Foundation's Perinatal Health and Wellness Program (PHWP) and KinderSmile Community Oral Health Centers (KSCOHC). Working closely with the PHWP Director, the Community Health Worker attends and represents KinderSmile Foundation at community, outreach, educational, maternal health, tabling, and recruitment events, acting as a liaison who links mothers and families to a permanent Dental Home for continuity of care.
This position requires an interest in public health and community service, along with excellent organizational, communication, and interpersonal skills. You are friendly, punctual, reliable, relatable, and have an affinity for working with children.
Essential Duties & Responsibilities
1. Community Engagement & Outreach: Attend and represent KinderSmile Foundation and PHWP at community, outreach, educational, maternal health, tabling, and recruitment events; participate in recruitment activities for PHWP patients, Dental Home families, and maternal agencies.
2. PHWP Liaison & Patient Linkage: Educate mothers from pregnancy through three years postpartum; act as liaison linking children, mothers, and families to a permanent Dental Home (Bloomfield, Newark, or Trenton) for continuity of care.
3. Patient & Maternal Health Education: Conduct Maternal and Infant Oral Health Education sessions covering the importance of oral health to maternal and infant outcomes, the safety of oral health care during pregnancy, and the value of preventive care.
4. Data Collection, Tracking & Auditing: Track and collect data for all patients linked to the Dental Homes; audit Eaglesoft chart entries; track recall-visit data and educational-program feedback.
5. Administrative & Reporting: Assist the PHWP Director and COO with data tracking and analysis; prepare ongoing reports; follow up with patients by phone and in Eaglesoft; help prepare informational materials and resources.
6. Dental Home On-Site Support: Greet and seat patients; prep and break down treatment rooms as needed; assist Dental Providers and staff; keep clinical and sterilization areas clean, neat, organized, and well stocked.
7. Professional Development: Train to independently conduct educational sessions for PHWP and become competent with all software tools utilized by KinderSmile Foundation programs.
Qualifications
Education & Credentials:
High school diploma or equivalent
Current BLS/CPR certification preferred
Experience:
Working experience in dental administration preferred
Minimum of 1–2 years of experience conducting community outreach, patient recruitment, and enrollment through health fairs, community events, tabling, and partnerships with community-based organizations or maternal health agencies
Experience connecting individuals and families to healthcare services and community resources
Experience working with pregnant or postpartum individuals, children, families, or vulnerable populations in a community health, maternal and child health, public health, or social services setting
Skills:
Good communication and telephone skills
Working knowledge of Google Workspace (Docs, Sheets)
Willingness to learn dental software and electronic health records
Bilingual (Spanish) required
Organized, punctual, reliable, and relatable
Works well as part of a team; enjoys working with children
