As health centers continue to advance integrated care models, leveraging health information technology (Health IT) is essential to strengthening coordination between medical and dental teams. This webinar will highlight practical strategies to advance oral health integration through interoperable electronic health record (EHR) systems, featuring two health centers that use AthenaOne and Epic. Presenters will share an overview of their organizations, including services, patient populations, and key drivers behind their integration efforts, and will discuss how they use Health IT tools to support referral pathways, shared scheduling, cross-team communication, and streamlined clinical workflows. The session will also explore how EHR functionalities, such as templates and documentation practices, enhance care coordination and team collaboration. In addition, speakers will highlight how data is captured and used within their systems to measure integration efforts, inform decision-making, and drive continuous quality improvement.
